Care finders arrange meetings to understand each person’s unique situation, provide specialist support to navigate and access aged care, and link with other community services to improve quality of life. A care finder can assist with arranging first-time aged care services, or with changing existing services to address new needs. A care finder can help by talking to My Aged Care on someone’s behalf and arranging an assessment, attending and providing support at the assessment, finding and short-listing aged care providers in the local area, completing forms and explaining aged care service agreements, checking in when services have started, to make sure everything is okay, accessing additional services such as mental health support, housing and homelessness support, and relevant community groups.
